Answer the following questions using this scale:
0 = not at all 1 = a little bit 2 = moderately 3 = quite a bit 4 = extremely
Little interest or pleasure in doings things
Feeling down, depress or hopeless
Trouble falling/staying asleep, sleeping too much
Poor appetite or overeating
Feeling bad about yourself, feeling that you are a failure, or that you have let yourself or your family down
Trouble concentrating on things, such as reading or watching T.V.
Thoughts that you would be better off dead, or of hurting yourself in some way.
